Applying for vacancies using the personal details application form

Use this page to apply for vacancies that ask for the personal details application form. Fill in the personal details application form and equal opportunities form and email or post them both to us. You will need to save the forms on your computer before you start filling them in.

The forms are available in alternative formats, such as Braille or audio. If you need an alternative format, please contact the person named in the job advert.

Equal opportunities form

We take equal opportunities very seriously. This form helps us to find out if our policy is working in practice.

Then, fill in the equal opportunities form (63.3KB RTF)

  1. Save your completed personal details application form and equal opportunities form to your computer.
  2. Make sure that the subject line of your email contains the job reference number and your full name. Please note: we cannot accept applications without the job reference number in the subject line of the email.
  3. Attach both your completed forms and email them to us at the email address given in the vacancy details.  

Posting your application

You can post your application to us, if you prefer.

Print off your completed personal details application form and equal opportunities form and post these to us at the address shown in the job advert. Or, to receive printed copies of the forms and guidance, please contact the person named in the job advert.
