The GP may enclose correspondence from the hospital that confirms diagnosis. |
| Hospital FR | The best source of information will be the hospital based specialist nurse or the consultant haemato-oncologist. They will be able to provide details of current treatment and confirm disabling effects. They are the best source of information on duration of planned treatment and prognosis of disabling effects. The most important information to check will be: Confirm diagnosis Confirm treatment is being received – e.g. some type of chemotherapy. This will be sufficient in most cases. Confirm severe disability due to specific side effects of therapy if claimed – e.g. walking difficulties due to acute cerebellar syndrome. Such damage often resolves quickly, even when severe – always ask for prognosis of side effects when requesting information about them. |
