Disability and Carers Service claim forms
To obtain Disability and Carers Service forms, please order from iON.
Please note that Business Reply Envelopes are no longer supplied with claim forms. If you require these envelopes, please contact your local Disability Benefit Centre.
Healthcare professionals – Med 3, Med 10 and MATB1
To order copies of the Med 3, Med 10 or MATB1 do not fill out the iON order form below. Instead complete your order using stationery that includes your surgery or practice details, for example, stationery with a letterhead.
Med 3 and Med 10 are available in pads of 50 and MATB1 in pads of 20. Med 3s are also available as sheets.
Please say how many pads (or Med 3 sheets) you need and provide a contact name for delivery. A product code is not required. Delivery takes approximately five working days.
