If you require fewer than 50 copies of a DWP leaflet, you can order online from the DWP information line service. When entering your details, please include which organisation you are representing.

To obtain Disability and Carers Service forms, please order from iON.
Please note that Business Reply Envelopes are no longer supplied with claim forms. If you require these envelopes, please contact your local Disability Benefit Centre.
Statement of Fitness for Work (Form Med 3)
To order copies of the Form Med 3 do not fill out the iON order form below.
Instead complete your order using stationery with your surgery or practice letterhead.
Fax or post your order to iON using the details below. Please specify the quantity of Med 3 sheets or Med 3 pads required. A product code is not required for these items.
