How to order products
This section gives you information on how to order free copies of DWP leaflets, including Disability and Carers Service products and products in Braille.
To get fewer than 50 copies of standard English and Welsh leaflets please contact your local Jobcentre Plus office. For your nearest office look under Jobcentre Plus, Jobcentre or social security office in the phone book.
To order larger quantities of any item – please complete the iON order form (474KB) 
and send it to iON.

You can order selected pensions related products from The Pension Service request online service.
Statement of Fitness for Work (Form Med 3)
To order copies of the Form Med 3 do not fill out the iON order form above.
Instead, please provide iON with either an NHS purchase order or complete your order using stationery with your surgery or practice letterhead.
Fax or post your order to iON using the details above. Please specify the quantity of Med 3 sheets or Med 3 pads required. A product code is not required for these items.
