18. I have made a telephone claim to Personal Independence Payment, what happens now?

Once we have established that you meet the entitlement conditions we’ll send you a form called ‘How your Disability affects you’ so you can tell us  how your condition affects your daily life, both on good and bad days and over a range of activities.

You can also send supporting evidence such as a letter from your GP with this form, which you send to DWP by post. You may find it helpful to keep a diary – write down a list of things you’ve needed help with or found difficult over a number of days. If the impact of your health condition or disability varies over time, you may want to keep a record of this. You can send in the diary with your form if you want to. You don’t have to fill in the form in one go. Take your time and describe all the help you need.

19. I have received a form to complete but I need some help. Who should I contact?

You may ask someone, such as a family member or customer representative group such as CAB to help you complete the form.

DWP is providing advice and training to customer representative groups to ensure that they understand how Personal Independence Payment works so that they can help with form completion.

20. I have posted the completed ‘How your Disability affects you’ form back to DWP. What happens now?

The form and any supporting evidence you have given us will be passed to a health professional who will review the information you have provided. They may ask your GP or any other contacts you’ve given for additional medical evidence.

You will be asked to attend a face-to-face consultation unless a decision can be made on the written evidence you have provided. If a face to face meeting is needed then the health professional will contact you direct to arrange it.
