a comprehensive web-based information store for older people, carers and professionals. - Older People and Social Isolation Pack (182KB)
a resources pack developed by front-line, multi-disciplined professionals whose work involves contact with socially isolated people. Applying principles of holistic working, it includes information, signposting, training and best practice. - Streamlined Assessment Pilot (183KB)
a joint venture between The Pension Service and the Local Authority Benefits Service in Leeds to explore ways to simplify the application process for benefits and pensions and referrals to supporting services.
Pilot evaluation report
As part of the project, all pilots undertook local evaluation of their pilot in addition to the national evaluation being undertaken by Warwick Business School.
